Kemptville District Hospital is a 40-bed acute care hospital located in the rapidly growing Eastern Ontario municipality of North Grenville, 40 minutes south of Ottawa. Our services include 24-hour emergency care, inpatient care, advanced orthopaedic surgery (as a satellite of The Ottawa Hospital), and outpatient care. We are an integrated health services organization with deep partnerships in our community and region. With our partners in the Ottawa West Four Rivers Ontario Health Team, we are collaborating on a new model of health care delivery that puts patients, families and caregivers at the centre of the health care system.

Heidi PeskettDischarge Planner/Home and Community Support Services Champlain Care Coordinator
My role is a combined role of Hospital Discharge Planner and Home and Community Support Services Champlain Care Coordinator. I meet with patients from the Inpatient Medical Unit, Convalescent Care, and the ER to assess eligibility and arrange Home and Community Care services, which may include nursing, personal support, physiotherapy, occupational therapy, speech language pathology, the dietitian, social work and long-term care planning. I also help connect patients with other community services including Kemptville Home Support, the Going Home Program, retirement homes and the Community Paramedic Program, to name a few.
I came to KDH because I liked the idea of the combined role of Discharge Planner and Care Coordinator. It is patient focused. The patient only has to speak to one person about their needs and concerns upon leaving the hospital and that person is the one to arrange services and make referrals to community support. In larger hospitals, this role is split between two people.
I also enjoy working in the small hospital setting. I have gotten to know and work closely with other team members (nurses, physiotherapists, occupational therapists, and physicians). We aim to improve the patient experience and hopefully provide the patient with a smooth transition home.

Last Tuesday morning I received a phone call from my teenage son Markus’s school informing me that he had inadvertently eaten a cookie containing peanuts. As he has a life-threatening peanut allergy, this was an immediate cause for concern. Markus was taken to the ER and immediately given a dose of norepinephrine, followed by an infusion after he had a second reaction. Markus was transferred to CHEO by ambulance later in the afternoon. As you can imagine, this was an incredibly impactful experience for Markus and my entire family. We have such respect for the wonderful work of the Emergency Team at KDH, to whom Markus owes his life. Everyone was caring, kind and professional. Special thanks go to Dr. Pelletier, Dr. Jones, and nurses Catherine, Heather, and Melissa."
